Wellness Nurse - OakView Health Center

Department: Assisted Living
Salary/Hourly Rate: $24 - $30 Per Hour


OakView Health Center is currently hiring a full or part time Wellness Nurse in our Assisted Living Department. The Wellness Nurse oversees that each resident's necessary care and services are provided determined by the assessment and individualized care and service plan. Assists the Assisted Living Director with maintaining compliance of Department of Social Services RCFE regulations. Supervise daily duties of caregivers and medication technicians. The Wellness Nurseís work hours will be based on the needs of the department and determined by the Director of Assistant Living. Compensation will be provided for on-call duty as necessary.


  • Initiates communication with physicians regarding maintenance and change of residentís health condition; implements new orders.
  • Communicates with families regarding the change of condition and needs of the residents.
  • Initiates and updates care and service plans quarterly as assigned under supervision of the Assisted Living Director and/or Administrator. Review assignment of resident units.
  • Complies with RCFE regulations and oversees documentation, notification and administration is completed and pertinent information is given and reported.
  • Responds timely to medical emergencies.       Oversees the completion of the incident report. Responsible for all required notifications including but not limited to: the residentís physician and residentís responsible party.  
  • Communicates with pharmacies regarding all new orders and cycled medications as necessary, verifies medication deliveries against physicianís orders when available.
  • Participates in Quality Improvement Committee efforts and initiatives to improve the quality of life, care and regulatory compliance.
  • Expected to return to the community in the event there is a disaster or emergency.
  • Responsible for obtaining durable medical equipment and insurance authorizations when necessary.
  • Assists with ordering of central supplies and its storage.
  • Assists with supervision and providing direction to Caregivers and Medication Technician staff. Submits staff performance information to Assisted Living Director to complete comprehensive annual and 90-day performance reviews.
  • Directs Caregivers/Med Techs to report any incidents and accidents, skin issues and any unusual occurrences immediately to ensure follow-up with AL Director, family, and physicians in a timely manner.
  • Assists Medication Technicians with review and correction of monthly medication sheets. Verifies medication supplies against medication record as necessary.

  • Promotes/protects resident rights.
  • Complies with abuse prevention and reporting policies and procedures.
  • Maintains compliance with federal, state and local regulatory requirements.
  • Participates in staff meetings and mandatory in-services as required.
  • Performs other work duties and responsibilities as assigned by supervisor.


  • Current licensure by the California Board of Vocational Nurse and Psychiatric Examiners required.
  • 1 year experience as Licensed Vocational Nurse as staff nurse in acute care setting desirable. 2 years in long term care with prior charge nurse experience preferred.
  • Excellent communication skills required (written and verbal). Good attention to detail and follow up skills required.
  • 1 year of supervisory experience preferred.
  • Current CPR and First Aid certification required.
  • Minimum 1 year of experience in an assisted living environment.
